Reasonable Adjustment Request

Use this form to request reasonable adjustments or to tell us about circumstances that affected an assessment, under the Reasonable Adjustments and Special Considerations Policy (DAIS-POL-012).

A reasonable adjustment is support put in place before an assessment so you can show what you know on a level footing (for example extra time or assistive software). Provide as much detail as you can. Your Internal Quality Assurer will review the request and confirm the centre decision with you.

This part is for special consideration, which is different. A reasonable adjustment is arranged before an assessment; special consideration is applied afterwards, when something outside your control affected how you did on the day. Bereavement, serious illness, injury, a platform or internet failure during a timed assessment, or extreme personal circumstances. Leave this section blank if it does not apply to you.

We ask for evidence where you can reasonably obtain it. Tell us now if you cannot, rather than delaying the request.
