Reasonable Adjustment Request
Use this form to request reasonable adjustments or to tell us about circumstances that affected an assessment, under the Reasonable Adjustments and Special Considerations Policy (DAIS-POL-012).
A reasonable adjustment is support put in place before an assessment so you can show what you know
on a level footing (for example extra time or assistive software). Provide as much detail as you can.
Your Internal Quality Assurer will review the request and confirm the centre decision with you.
